A bunch with gear list, all shot on a Nikon d200 with a Tokina 12-24:
Buffalo Auditorium, two Garrity spots, 10 minutes and a whole lot of running around. The bluish light coming from the top left side is the only natural light in the shot, coming through a crack in a boarded window. This is a composite of two shots, the two lights shining from the far end of ice level are from another exposure.

I think the technique here is pretty obvious. It's a Garrity spot if that matters.
Garrity spot, and a red flare (Kowalski's idea). Tip #1 for using flares in damp tunnel environments: stand upwind, and work fast - smoke backs up quickly, and takes a while to dissipate.
